ANC Waiver

I,

indemnify, hold harmless, and forever discharge Arts for Nations Center, and its agents, employees, officers, directors, affiliates, successors, managers, and assigns, from any and all claims, demands, debts, expenses, causes of action, lawsuits, damages, and liabilities of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to my/my child’s participation in any events or activities conducted by or on the premises of Arts for Nations Center.

I understand that the activities or functions in which I participate may be inherently dangerous and can cause serious injuries, including bodily injury, damage to personal property, and/or death. On behalf of myself/my child, I waive all claims for damages, injuries, and death sustained by me or my property that I may have against the aforementioned released party during such activity.

By this Waiver, I assume any risk, take full responsibility, and waive any claims of personal injury, death, or damage to personal property associated with Arts for Nations Center, including but not limited to using the facility and its equipment in any manner, form, or fashion, and practicing or engaging in Arts for Nations Center activities or related activities on and off the premises.

My signature is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law. I confirm that I am 18 years of age or older and mentally competent to enter into this Waiver.

Arts for Nations Center PHOTO/VIDEO RELEASE FORM

Students or company members of Arts for Nations Center may have the opportunity to be photographed or videotaped at various events. These photos or videos may be used in Arts for Nations Center publications, advertisements, website content, or on various social media platforms. All media materials are the property of Arts for Nations Center, and no financial compensation will be provided for the use of any photographs or videos.

I acknowledge that Arts for Nations Center may choose not to use my photo or video at this time, but reserves the right to do so at its discretion at a later date. Arts for Nations Center also reserves the right to discontinue the use of any photos or videos without prior notice.
